This agreement releases, waives, and discharges All Occasion Signs and/or their

owners, directors, officers, employees, volunteers, representatives, and agents; from all

liability relating to injuries that may occur during pickup, delivery, setup/breakdown,

loading/unloading equipment and while on the premises of 275 Miller Street in the town

of Lancaster, Village of Depew, State of New York, Zip code 14043 or while on

CLIENTS premises located on ________________________ in the town of

_____________ village of ______________, state of New York, Zip code

___________.

By signing this agreement, I AGREE to release All Occasion Signs and/or their

owners, directors, officers, employees, volunteers, representatives, and agents entirely

free from any liability, including but not limited to financial responsibility for injuries

incurred, regardless of whether injuries are caused by negligence or carelessness.

I also acknowledge the risks involved in this sign and lawn ornament rental. I am

made aware that heavy lifting may be involved in varying weather conditions. I AGREE

that I am participating voluntarily, and that all risks have been made clear to me.

Additionally, I do not have any conditions that will increase my likelihood of experiencing

injuries while engaging in this activity.

By signing below I forfeit all rights to bring a suit against All Occasion Signs and/or

their owners, directors, officers, employees, volunteers, representatives, and agents for

any reason. I will also make every effort to obey safety precautions as explained to me

verbally. I will ask for clarification when needed.

I understand I will receive a sign and lawn ornament rental for a total cost of

$________. I am aware that I am fully responsible for the payment to All Occasion

Signs for all equipment rented and assume full responsibility for rental cost.

Reimbursement for damaged, lost, or stolen signs or lawn ornaments must be paid

for in full IMMEDIATELY in CASH. Replacement cost for a sign is $200, replacement

cost for each lawn ornament is $25 (Cow lawn ornaments $40 each) (Storks $400

each). I AGREE to return all rented materials by _________________________.

Failure to do so will result in a late fee of $10 per day.

I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS

CONTENT. I AM AWARE THAT THIS IS A RENTAL CONTRACT AND A RELEASE OF

LIABILITY AND IS CONSIDERED A LEGALLY BINDING CONTRACT IN THE STATE

OF NEW YORK. I AM SIGNING THIS AT MY OWN FREE WILL.
